Thinking about and planning ahead for a time when we may not be able to make or communicate our own choices for care and medical treatment is called advance care planning.


If you or someone close to you has serious health problems, now is the right time to start advance care planning. Advance planning can help relieve the burden of substitute decision making for you, your family and loved ones.
